Event Waiver

Photo Release

The OTL has my permission photos of the above named participant to promote the library and its work. I understand that the images may be used in print publications, online publications, presentations, websites, and social media. I also understand that no royalty, fee, or other compensation shall become payable to me by reason of such use.

Understood, please identify yourself to our staff. We will do our best not to feature you (or the participant), but please alert us if you suspect you may be included in a shot.

Liability Waiver

I state that I will use the tools on site at the workshop in a safe, responsible, and prudent manner.

I do hereby for myself, on behalf of my successors and assigns, in consideration of being permitted to borrow tools, waive any and all, claims against Society for Social Ingenuity (SSI), also knows as the Ottawa Tool Library (OTL), their directors, officers, agents, volunteers, representatives, and employees for any injury or injuries of any nature that I or a third party may suffer or incur as a result of use of the tools that I am borrowing from the OTL.

I hereby for myself, on behalf of my successors and assigns, in consideration of being permitted to borrow tools, use the Ottawa Tool Library space, and work with the instructor(s), agree to release and indemnify and hold harmless the Society for Social Ingenuity (SSI) and the Ottawa Tool Library (OTL), their officers, agents, volunteers, representatives, and employees from and against any and all liability, losses, claims, demands, damages, or causes of action of any kind arising out of my borrowing of tools, using the Repair Café space and working with the fixers, including related to the death or injury of any persons, for any property damage suffered or incurred by any person, and for any court costs and reasonable legal fees.

I am aware that SSI, OTL, their partners, directors, officers, members, volunteers, representatives and employees claim no expertise and make no representation concerning the fitness of any tool for any particular use.

Liability Waiver (U18)

I certify that I am at least 18 years of age and I am the above-named parent/guardian with the legal responsibility for the participant named above, and I consent and agree to his/her release as provided above, and for myself, my heirs, successors, and assigns, I release and agree to indemnify SSI, operating as OTL, its directors, officers, agents, volunteers, representatives and employees from any and all liabilities relating to this participant’s activities on any premises occupied by the OTL and/or the use of any Equipment/Materials. I confirm that I have read the above document before signing and I fully understand its contents, meaning and impact.
